If you have a child entering preschool, kindergarten, sixth grade, or transferring to Ballard County Schools for the 2022-23 school year, the attached health requirements must be met to comply with Kentucky state regulations and will be strictly enforced. Parents will be asked to provide a Social Security number and birth certificate for each child.
TRANSFER STUDENTS must present a valid Kentucky School Medical Examination form (physical exam) and Kentucky Immunization Certificate.
